Percutaneous coronary revascularization for myocardial infarction in chronic kidney disease: recognizing the risk while seizing the benefits.
نویسنده
چکیده
Chronic kidney disease (CKD) with an estimated glomerular filtration rate 60 ml/min is seen in over 30% of patients undergoing percutaneous coronary intervention (PCI) (1). The prevalence of CKD continues to rise, in part because of increasing diabetes and hypertension (2). In patients undergoing PCI for either stable coronary artery disease or acute myocardial infarction, CKD is one of the strongest risk factors for shortand long-term mortality (1,3).

Management of acute coronary syndrome in patients with chronic kidney disease: if we don't risk anything, we risk even more.
The utilization of coronary angiography and percutaneous coronary intervention in the routine care for patients with acute coronary syndromes (ACS) has resulted in significant improvement in their prognosis. Consequently, recommendations on the use of these management strategies are reflected in the ACC/AHA guidelines for the management of patients with unstable angina and myocardial infarction...
متن کاملManagement of coronary artery disease in patients with chronic kidney disease.
Patients with chronic kidney disease (CKD) are considered to belong to the highest risk group for the development of cardiovascular events. These patients should be subject to aggressive risk-factor modification. However, management of coronary artery disease in patients with CKD can be uniquely challenging. Many of the medications used in the treatment and prevention of coronary artery disease...
متن کاملMode of Coronary Revascularization and Short term Clinical Outcomes in Patients with Chronic Kidney Disease
BACKGROUND AND OBJECTIVE Percutaneous coronary intervention (PCI) and coronary artery bypass graft (CABG) surgery are two alternative methods for coronary revascularization, but it remains controversial as which one is associated with lower risks of worse clinical outcomes for chronic kidney disease (CKD) patients. We determined the mode of coronary revascularization (PCI vs. CABG) which is ass...
متن کاملNo-Reflow Phenomenon in Patients with ST-Elevation Acute Myocardial Infarction, Treated with Primary Percutaneous Coronary Intervention: A Study of Predictive Factors
Introduction: No-reflow phenomenon in coronary vessels, manifested in some patients with reperfused acute myocardial infarction (MI), is associated with poor clinical and functional outcomes. Therefore, evaluation of predisposing risk factors can be helpful in risk assessment and identification of patients at higher risk. Herein, we aimed to study the predictive factors for the development of...
متن کاملChronic Total Occlusion Percutaneous Coronary Intervention: Evidence and Controversies
C oronary chronic total occlusions (CTOs) are defined as 100% occlusions with TIMI (Thrombolysis in Myocardial Infarction) 0 flow with at least a 3-month duration. Treatment options for patients with coronary CTOs include lifestyle changes and medications (as is appropriate for all patients with coronary artery disease) and coronary revascularization with either percutaneous coronary interventi...
